Liverpool City Council is seeking an experienced and compassionate Adult Mental Health Social Worker to join their dedicated team. Key Responsibilities:

• Manage a caseload of adults with mental health conditions and complex needs.
• Conduct care assessments, mental capacity assessments, and risk assessments.
• Develop and implement care plans and support packages tailored to individual needs.
• Work closely with multi-disciplinary teams to ensure holistic support for service users.
• Provide care management, including coordinating services and reviewing care plans.

Requirements:

• Qualified Social Worker with experience in adult mental health.
• In-depth knowledge of mental health legislation, including the Care Act and Mental Health Act.
• Strong assessment, analytic, and decision-making skills.
• Ability to work independently while collaborating with other professionals to ensure the best outcomes for individuals.
